ABSTRACT

Objective: To evaluate the diagnostic power of SARC-F and SARC-CalF as screening tools for sarcopenia risk in older adults with type 2 diabetes mellitus. Methods: This cross-sectional study of 128 patients was conducted at the endocrinology outpatient clinic of a hospital in Recife, Brazil between July 2022 and February 2023. Sarcopenia was diagnosed according to original and updated European Consensus criteria for older adults. Muscle mass was assessed with electrical bioimpedance, muscle strength was assessed with a handgrip test, and physical performance was assessed with gait speed. Sarcopenia risk was assessed using the SARC-F and SARC-CalF instruments. The sensitivity, specificity, positive and negative predictive values, receiver operating characteristic curve, and area under the curve were analyzed to determine the best diagnostic performance. Results: According to the original and updated versions of the European Consensus criteria, the prevalence of sarcopenia was 25.00% and 10.90%, respectively. Sarcopenia risk was 17.20% according to the SARC-F and 23.40% according to the SARC-CalF. The sensitivity and specificity of the SARC-F ranged from 12.55% to 36.11% and 71.87% to 92.39%, respectively, while those of the SARC-CalF ranged from 47.22% to 85.71% and 82.46% to 88.89%, respectively. The area under the curve for the SARC-F and SARC-CalF varied between 0.51 and 0.71 and 0.67 and 0.86, respectively. Conclusions: The SARC-CalF had better diagnostic performance for all analyzed diagnostic criteria. Adding calf circumference to the SARC-F was an effective screening method for sarcopenia risk in the study population


Objetivo: Avaliar o poder diagnóstico do SARC-F e SARC-CalF como ferramentas de rastreamento para o risco de sarcopenia em idosos com diabetes mellitus tipo 2. Metodologia: Estudo transversal com 128 pacientes desenvolvido no ambulatório de endocrinologia de um hospital do Recife entre julho de 2022 e fevereiro de 2023. A sarcopenia foi diagnosticada de acordo com os critérios do Consenso Europeu para sarcopenia em pessoas idosas e sua versão atualizada. Foi realizada bioimpedância elétrica para avaliar a massa muscular, teste de preensão palmar para a força muscular e teste de velocidade de marcha para a performance física. O risco para sarcopenia foi avaliado por meio do SARC-F e SARC-CalF. Realizou-se análise de sensibilidade, especificidade, valores preditivos positivos e negativos, curva Característica de Operação do Receptor (ROC) e área sob a curva (AUC) para determinar a melhor performance diagnóstica. Resultados: A prevalência de sarcopenia foi de 25,00% de acordo com a primeira versão do Consenso Europeu e 10,90% considerando a versão atualizada. O risco para sarcopenia foi de 17,20% (SARC-F) e 23,40% (SARC-CalF). A sensibilidade do SARC-F variou entre 12,55 e 36,11%, e a espec ificidade entre 71,87 e 92,39%, enquanto o SARC-CalF apresentou sensibilidade entre 47,22 e 85,71% e especificidade entre 82,46 e 88,89%. A AUC do SARC-F variou entre 0,51 e 0,71, enquanto o SARC-CalF ficou entre 0,67 e 0,86. Conclusões: O SARC-CalF apresentou melhor performance diagnóstica quando comparado a todos os critérios diagnósticos analisados. A adição da circunferência da panturrilha é um método eficaz para o rastreamento do risco de sarcopenia na população do estudo

Abstract Background: Sarcopenia is a disease that involves skeletal muscle mass loss and is highly prevalent in the older adult population. Moreover, the incidence of sarcopenia is increased in patients with hypertension. Objectives: The study aimed to evaluate the association between the classes of the drugs used for arterial hypertension treatment and the presence or absence of sarcopenia. Methods: 129 older adults with hypertension were evaluated by the researchers who registered the participants medication for arterial hypertension treatment. Sarcopenia level was measured by anthropometric parameters, muscular strength, and functional capacity. The data were analyzed by one-way ANOVA followed by post-hoc test and Fisher's exact test; statistical significance was set at 0.05. Results: Age was not different between women with different levels of sarcopenia, but significant differences were observed between men with absent sarcopenia (66.8±4.2 years) and men with probable sarcopenia (77.0±10.2 years). Individuals with absent sarcopenia showed higher handgrip strength (men: 33.8±7.4, women: 23.2±4.6 Kgf) in comparison with those with sarcopenia (men with probable sarcopenia: 9.5±3.3 Kgf, women with probable, confirmed, and severe sarcopenia: 11.7±2.5, 12.2±3.0, 11.8±1.8 Kgf, respectively). The analysis showed an association between the type of medication and degree of sarcopenia; diuretics were significantly associated with probable sarcopenia, and angiotensin II receptor blockers (alone or in combination with diuretics) was associated with absence of sarcopenia. Conclusion: In conclusion, handgrip strength was a good method to diagnose sarcopenia, and diuretics were associated with increased risk of sarcopenia in older adults with hypertension.

ABSTRACT Objective: To evaluate the prevalence of osteosarcopenia and the association of osteosarcopenia with trabecular bone score (TBS) in a group of patients with type 2 diabetes mellitus(T2DMG) compared with a paired control group (CG). Materials and methods: Cross-sectional study with men and women ≥ 50 years recruited by convenience. Patients in both groups answered questionnaires and underwent evaluation of bone mineral density (BMD), handgrip strength (HGS), and TBS. The T2DMG also underwent a gait speed (GS) test. Sarcopenia was defined as low lean mass plus low HGS or GS according to the Foundation for the National Institute of Health Sarcopenia Project, and osteosarcopenia was deemed present when sarcopenia was associated with osteopenia, osteoporosis, or low-energy trauma fractures. Results: The T2DMG (n = 177) and CG (n = 146) had, respectively, mean ages of 65.1 ± 8.2 years and 68.8 ± 11.0 years and 114 (64.4%) and 80 (54.7%) women. T2DMG versus the CG had higher rates of osteosarcopenia (11.9% versus 2.14%, respectively, p = 0.010), sarcopenia (12.9% versus 5.4%, respectively, p < 0.030), and fractures (29.9% versus 18.5%, respectively, p = 0.019), and lower HGS values (24.4 ± 10.3 kg versus 30.9 ± 9.15 kg, respectively, p < 0.001), but comparable BMD values. Mean TBS values were 1.272 ± 0.11 and 1.320 ± 0.12, respectively (p = 0.001). On multivariate analysis, age, greater waist circumference, fractures, and osteoporosis increased the risk of degraded TBS. Osteosarcopenia was associated with diabetes complications (p = 0.03), calcium and vitamin D supplementation (p = 0.01), and all components of osteosarcopenia diagnosis (p < 0.05). Conclusion: Compared with the CG, the T2DMG had a higher prevalence of osteosarcopenia, sarcopenia, and fractures and lower bone quality assessed by TBS.

La sarcopenia es uno de los principales síndromes geriátricos que está asociada con limitación en las actividades de la vida diaria, discapacidad y mortalidad en la población adulta mayor. Sin embargo, su definición, criterios diagnósticos y puntos de corte son diversos en la literatura, lo que dificulta el diagnóstico de la sarcopenia. El objetivo de esta revisión narrativa fue detectar en la literatura todos los consensos internacionales relacionados con sarcopenia, comparar su definición, criterios diagnósticos y puntos de corte, con el propósito de identificar una definición y criterios que puedan ser utilizados en Latinoamérica. Fueron identificados 7 consensos internacionales que han definido la sarcopenia como un síndrome/enfermedad caracterizado por la pérdida de la masa y función muscular asociada al proceso de envejecimiento. A pesar de tener una definición similar, no hay una total concordancia entre los criterios y evaluaciones. La sarcopenia puede ser detectada utilizando evaluaciones funcionales de fuerza muscular, velocidad de la marcha y equilibrio, que son frecuentemente utilizados por profesionales del área de la rehabilitación. Adicionalmente, el riesgo de sarcopenia puede ser identificado utilizando los cuestionarios SARC-F y SARC-CalF. Debido a su impacto funcional, la sarcopenia debería ser diagnosticada precozmente en adultos mayores por terapeutas ocupacionales, kinesiólogos y fisioterapeutas, para prevenir y tratar futuras complicaciones clínicas y funcionales en esta población. Es necesario la creación de valores de referencia para favorecer el diagnóstico de la sarcopenia en población latinoamericana.


Sarcopenia is one of the main geriatric syndromes that has been associated with limitation in activities of daily living, disability, and mortality in the elderly population. However, the definition, diagnostic criteria and cut-off points are diverse in the literature, which makes the diagnosis of sarcopenia difficult. The aim of this narrative review was to detect in the literature all the international consensus related to sarcopenia, to compare their definition, diagnostic criteria and cut-off points, in order to identify a definition and criteria that can be used in Latin America. Seven international consensuses were identified that have defined sarcopenia as a syndrome / disease characterized by loss of muscle mass and function associated with the aging process. Despite having a similar definition, there is not a total concordance between the criteria and evaluations. Sarcopenia can be detected using functional assessments of muscle strength, gait speed, and balance, which are frequently used by professionals in the area of rehabilitation. Additionally, the risk of sarcopenia can be assessed using SARC-F and SARC-CaF. Due to its functional impact, sarcopenia should be diagnosed early in older adults by occupational therapists, physical therapists and physiotherapists, to prevent and treat future clinical and functional complications in this population. The creation of reference values is necessary to favor the diagnosis of sarcopenia in the Latin American population.

Introducción: El progresivo envejecimiento poblacional se estima a nivel mundial como un indicador de mejora de la salud. No obstante, al vivir más años se le asocian problemas sanitarios que se van posicionando en el tiempo, y uno de ellos es la sarcopenia, la que se considera una condición común en los adultos mayores y se asocia a la inactividad física, baja resistencia, baja velocidad de marcha y disminución de la movilidad. Estos factores contribuyen a un aumento del riesgo de caída y a una disminución de la funcionalidad y de la calidad de vida. Objetivo: Mostrar evidencia actualizada respecto a los resultados del entrenamiento de fuerza con restricción parcial del flujo sanguíneo en población adulto mayor con sarcopenia, como estrategia costo-efectiva para la dependencia asociada al envejecimiento. Métodos: Se realizó una búsqueda en bases de datos científicas EBSCOhost, ScienceDirect, Web of Science y en el buscador PubMed. Se utilizaron los siguientes términos de búsqueda: entrenamiento con restricción parcial del flujo sanguíneo, sarcopenia, adulto mayor. Los artículos analizados incluyeron revisiones y artículos de investigación, principalmente ensayos clínicos controlados y revisiones sistemáticas con metanálisis. Conclusiones: El entrenamiento con restricción parcial del flujo sanguíneo en el adulto mayor surge como una herramienta útil para intervenir en la sarcopenia asociada al envejecimiento, constituyendo una alternativa para inducir la ganancia de fuerza muscular, con la disminución de los riesgos del entrenamiento de alta intensidad. Además, es una técnica económica y fácil de implementar en centros de salud de asistencia masiva(AU)


Introduction: Progressive population aging is estimated globally as an indicator of health improvement. However, when living is extended, health issues appear; one of them is sarcopenia, which is consider a common condition in older adults and it is associated to physical inactivy, low resistance, low walk speed and a decrease in mobility. These factors contribute to an increase of falls risk and to a decrease of functionality and life quality. Objective: To show updated evidence on the results of the strength training with partial restriction of blood flow in older adults populations with sarcopenia, as a cost-effective strategy for dependence associated to aging. Methods: Search was done in scientific databases as EBSCOhost, ScienceDirect, Web of Science and PubMed searcher. Following terms were used: training with partial restriction of blood flow, sarcopenia, older adult. The analyzed articles included reviews and research articles, mainly controlled clinical trials and systematic reviews with meta-analysis. Conclusions: Training with partial restriction of the blood flow in older adults emerges as an useful tool to intervene in sarcopenia associated to aging, being an alternative to induce muscle force´s gain, and decreasing the risks of high intensity training. In addition, it is an economic and easy to implement technique in health centers with massive audiences(AU)

ABSTRACT Purpose The present study aimed to determine whether sarcopenia after radical cystectomy (RC) could predict overall survival (OS) in patients with urothelial bladder cancer (UBC). Materials and Methods The lumbar skeletal muscle index (SMI) of 80 patients was measured before and 1 year after RC. The prognostic significance of sarcopenia and SMI decrease after RC were evaluated using Kaplan-Meier analysis and a multivariable Cox regression model. Results Of 80 patients, 26 (32.5%) experienced sarcopenia before RC, whereas 40 (50.0%) experienced sarcopenia after RC. The median SMI change was -2.2 cm2/m2. Patients with sarcopenia after RC had a higher pathological T stage and tumor grade than patients without sarcopenia. Furthermore, the overall mortality rate was significantly higher in patients with sarcopenia than in those without sarcopenia 1 year after RC. The median follow-up time was 46.2 months, during which 22 patients died. Kaplan-Meier estimates showed a significant difference in OS rates based on sarcopenia (P=0.012) and SMI decrease (P=0.025). Multivariable Cox regression analysis showed that SMI decrease (≥2.2 cm2/m2) was an independent predictor of OS (hazard ratio: 2.68, confidence interval: 1.007-7.719, P = 0.048). Conclusions The decrease in SMI after surgery might be a negative prognostic factor for OS in patients who underwent RC to treat UBC.

ABSTRACT Background: Sarcopenia is prevalent before liver transplantation, and it is considered to be a risk factor for morbidity/mortality. After liver transplantation, some authors suggest that sarcopenia remains, and as patients gain weight as fat, they reach sarcopenic obesity status. Aim: Prospectively to assess changes in body composition, prevalence and associated factors with respect to sarcopenia, obesity and sarcopenic obesity after transplantation. Methods: Patients were evaluated at two different times for body composition, 4.0±3.2y and 7.6±3.1y after transplantation. Body composition data were obtained using bioelectrical impedance. The fat-free mass index and fat mass index were calculated, and the patients were classified into the following categories: sarcopenic; obesity; sarcopenic obesity. Results: A total of 100 patients were evaluated (52.6±13.3years; 57.0% male). The fat-free mass index decreased (17.9±2.5 to 17.5±3.5 kg/m2), fat mass index increased (8.5±3.5 to 9.0±4.0; p<0.05), prevalence of sarcopenia (19.0 to 22.0%), obesity (32.0 to 37.0%) and sarcopenic obesity (0 to 2.0%) also increased, although not significantly. The female gender was associated with sarcopenia. Conclusion: The fat increased over the years after surgery and the lean mass decreased, although not significantly. Sarcopenia and obesity were present after transplantation; however, sarcopenic obesity was not a reality observed in these patients.


RESUMO Racional: A sarcopenia é prevalente antes do transplante de fígado e é considerada fator de risco para morbidade/mortalidade desses pacientes. Após o transplante hepático, alguns autores sugerem que a sarcopenia permanece, e os pacientes ganham peso na forma de gordura, atingindo o status de obesidade sarcopênica. Objetivo: Avaliar prospectivamente as mudanças na composição corporal, prevalência e fatores associados em relação à sarcopenia, obesidade e obesidade sarcopênica após o transplante. Métodos: Os pacientes foram avaliados em dois momentos diferentes para composição corporal, 4,0±3,2 e 7,6±3,1 anos e após o transplante. Os dados da composição corporal foram obtidos por meio de bioimpedância elétrica. O índice de massa livre de gordura e o índice de massa gorda foram calculados, e os pacientes foram classificados nas seguintes categorias: sarcopênico; obesidade; obesidade sarcopênica. Resultados: Foram avaliados 100 pacientes (52,6±13,3 anos; 57,0% homens). A prevalência de sarcopenia (19,0% para 22,0%), obesidade (32,0% para 37,0%) e índice de massa livre de gordura (17,9±2,5 para 17,5±3,5 kg/m2), índice de massa gorda aumentou (8,5±3,5 para 9,0±4,0 kg/m2), e obesidade sarcopênica (0 para 2,0%) também aumentaram, embora não significativamente. O gênero feminino foi associado à sarcopenia. Conclusão: Após a operação, a gordura aumentou ao longo dos anos e a massa magra diminuiu, embora não significativamente. A sarcopenia e a obesidade estavam presentes após o transplante; no entanto, a obesidade sarcopênica não foi realidade observada nesses pacientes.

RESUMO: Objetivo: Estimar a prevalência e os fatores associados à sarcopenia, dinapenia e sarcodinapenia em idosos residentes no município de São Paulo. Métodos: Estudo transversal de base populacional envolvendo 1.168 idosos pertencentes à terceira onda do Estudo SABE (Saúde, Bem-Estar e Envelhecimento), em 2010. Foramconsiderados sarcopênicos os idosos com índice de massa muscular esquelética ≤ 8,90 kg/m2 para homens e ≤6,37 kg/m2 para mulheres, dinapênicos aqueles com força de preensão manual < 30 kg para homens e < 20kg para mulheres, e sarcodinapênicos aqueles que apresentavam sarcopenia associada à dinapenia. Características sociodemográficas, comportamentais, condições clínicas, nutricionais e bioquímicas foram consideradas para determinar os fatores associados a cada uma das três condições por meio de regressão logística multinomial. Resultados: A prevalência de sarcopenia, dinapenia e sarcodinapenia foi, respectivamente, 4,8% (IC95% 3,6-6,3), 30,9% (IC95% 27,5-34,6) e 9,0% (IC95% 7,2-11,3). O avanço da idade e a desnutrição foram associados às três condições analisadas. O prejuízo cognitivo foi associado à dinapenia e à sarcodinapenia. A escolaridade, ter o hábito de fumar e não ter vida conjugal foram associados à sarcopenia, enquanto osteoartrite, escolaridade, ser ex-fumante e apresentar valores baixos de hemoglobina foram associados à dinapenia. Foram associados à sarcodinapenia o hábito de fumar e o risco de desnutrição. Conclusão: Dinapenia é a condição mais prevalente na população idosa, seguida pela sarcodinapenia e sarcopenia. Exceto por idade, escolaridade e desnutrição, os fatores associados à sarcopenia e à dinapenia são distintos. Entretanto, há similaridades em algumas associações quando se trata da presença de sarcodinapenia.


ABSTRACT: Objectives: To estimate the prevalence of sarcopenia, dynapenia, and sarcodynapenia and associated factors in older adults in the city of São Paulo, Brazil. Methods: A population-based, cross-sectional study was conducted with 1,168 older adults who participated in the third wave of the Health, Well-being, and Aging study in 2010 (SABE study). Men and women with skeletal muscle mass ≤ 8.90 and ≤ 6.37 kg/m2, respectively, were considered sarcopenic. Men and women with grip strength < 30 and < 20 kg, respectively, were considered dynapenic. Those with both conditions were considered sarcodynapenic. Sociodemographic, behavioral, clinical, nutritional, and biochemical characteristics were investigated as factors associated with each of the three conditions using multinomial logistic regression. Results: Theprevalence of sarcopenia, dynapenia, and sarcodynapenia was 4.8% (95%CI 3.6 - 6.3), 30.9% (95%CI 27.5 - 34.6) and 9.0% (95%CI 7.2-11.3), respectively. An increase in age and malnutrition was associated with all the three conditions. Cognitive impairment was associated with both dynapenia and sarcodynapenia. Schooling, current smoking habit, and not having a marital life were associated with sarcopenia. Osteoarthritis, schooling, being an ex-smoker, and low hemoglobin were associated with dynapenia. Smoking habit and the risk of malnutrition were associated with sarcodynapenia. Conclusion: Dynapenia is more prevalent among older adults, followed by sarcodynapenia, and sarcopenia. With the exception of age, schooling, and malnutrition, the factors associated with sarcopenia and dynapenia are different. However, there are similarities in some associations regarding the presence of sarcodynapenia.

Resumo: O objetivo do presente estudo foi verificar a prevalência de sarcopenia e sua associação com mudanças nos fatores socioeconômicos, comportamentais e de saúde em idosos. O trabalho, longitudinal e de base populacional, incluiu 598 idosos (≥ 60 anos) de Florianópolis, Sul do Brasil. A sarcopenia foi identificada por meio do índice de massa muscular apendicular (IMMA), de acordo com o sexo (IMMA < 7,26kg/m² para homens e < 5,5kg/m² para mulheres). Foram avaliadas as mudanças ocorridas entre os dois momentos do estudo (2009/2010 e 2013/2014), relativas aos fatores socioeconômicos, comportamentais e de saúde. Foram realizadas análises de regressão logística bruta e ajustada. A prevalência de sarcopenia foi de 17% (IC95%: 12,4-22,9) nas mulheres e de 28,8% (IC95%: 21,3-37,7) nos homens. No modelo final, as mulheres que se mantiveram ingerindo ou passaram a ingerir álcool (OR = 0,31; IC95%: 0,11-0,91) apresentaram menor chance de ter sarcopenia. Aquelas que permaneceram fumando ou passaram a fumar (OR = 2,55; IC95%: 1,16-5,58) e se mantiveram ou passaram a ser insuficientemente ativas (OR = 2,90; IC95%: 1,44-5,84) apresentaram mais chance de ter sarcopenia. Para os homens, nenhuma variável de mudança foi associada a sarcopenia. Os resultados sugerem que manter-se fumando ou passar a ter o hábito de fumar, assim como permanecer ou passar a ser inativo fisicamente, são fatores de risco preveníveis e modificáveis para a sarcopenia.


Resumen: El objetivo del presente estudio fue verificar la prevalencia de sarcopenia y su asociación con cambios en factores socioeconómicos, comportamentales y de salud en ancianos. El trabajo, longitudinal y de base poblacional, incluyó a 598 adultos mayores (≥ 60 años) de Florianópolis, sur de Brasil. La sarcopenia se identificó mediante el índice de masa muscular apendicular (IMMA), de acuerdo con el sexo (IMMA < 7,26kg/m² para hombres y < 5,5kg/m² para mujeres). Se evaluaron los cambios ocurridos entre los dos momentos del estudio (2009/2010 y 2013/2014), referentes a factores socioeconómicos, comportamentales y de salud. Se realizaron análisis de regresión logística bruta y ajustada. La prevalencia de sarcopenia fue de 17% (IC95%: 12,4-22,9) en las mujeres y de 28,8% (IC95%: 21,3-37,7) en los hombres. En el modelo final, las mujeres que se mantuvieron ingiriendo o empezaron a ingerir alcohol (OR = 0,31; IC95%: 0,11-0,91) presentaron una menor oportunidad de sufrir sarcopenia. Aquellas que permanecieron fumando o empezaron a fumar (OR = 2,55; IC95%: 1,16-5,58) y se mantuvieron o empezaron a ser insuficientemente activas (OR = 2,90; IC95%: 1,44-5,84) presentaron más oportunidad de sufrir sarcopenia. Para los hombres, ninguna variable de cambio se asoció a la sarcopenia. Los resultados sugieren que mantenerse fumando o pasar a tener el hábito de fumar, así como permanecer o pasar a estar inactivo físicamente, son factores de riesgo prevenibles y modificables en el caso de la sarcopenia.


Abstract: This study aimed to verify the prevalence of sarcopenia and its association with changes in socioeconomic, behavioral, and health factors in the elderly. The longitudinal population-based study included 598 elderly (≥ 60 years) in Florianópolis, Santa Catarina State, in the South of Brazil. Sarcopenia was defined on the basis of appendicular skeletal mass index (ASMI) according to gender (ASMI < 7.26kg/m² for men and < 5.5kg/m² for women). We assessed changes that occurred between the two study waves (2009/2010 and 2013/2014) in relation to socioeconomic, behavioral, and health factors. Crude and adjusted logistic regression analyses were performed. Prevalence of sarcopenia was 17% in women (95%CI: 12.4-22.9) and 28.8% in men (95%CI: 21.3-37.7). In the final model, women that continued to consume or that started consuming alcohol (OR = 0.31; 95%CI: 0.11-0.91) showed lower odds of sarcopenia. Women who continued to smoke or that started smoking (OR = 2.55; 95%CI: 1.16-5.58) and/or that remained inactive or became insufficiently active (OR = 2.90; 95%CI: 1.44-5.84) showed higher odds of sarcopenia. For men, no change variable was associated with sarcopenia. The results suggest that continuing or starting to smoke and remaining or becoming physically inactive are preventable and modifiable risk factors for sarcopenia.

ABSTRACT

Background Evidence suggests a nutritional transition process in patients with inflammatory bowel disease. Obesity, which was once an uncommon occurrence in such patients, has grown in this population at the same prevalence rate as that found in the general population, bringing with it an increased risk of cardiovascular disease. Objective The aim of the present study was to determine the nutritional status and occurrence of cardiovascular risk factors in patients with inflammatory bowel disease. Methods A case-series cross-sectional study was conducted involving male and female adult outpatients with inflammatory bowel disease. Data were collected on demographic, socioeconomic, clinical and anthropometric variables as well as the following cardiovascular risk factors: sedentary lifestyle, excess weight, abdominal obesity, medications in use, comorbidities, alcohol intake and smoking habits. The significance level for all statistical tests was set to 5% (P< 0.05). Results The sample comprised 80 patients with inflammatory bowel disease, 56 of whom (70.0%) had ulcerative colitis and 24 of whom (30.0%) had Crohn's disease. Mean age was 40.3±11 years and the female genre accounted for 66.2% of the sample. High frequencies of excess weight (48.8%) and abdominal obesity (52.5%) were identified based on the body mass index and waist circumference, respectively, in both groups, especially among those with ulcerative colitis. Muscle depletion was found in 52.5% of the sample based on arm muscle circumference, with greater depletion among patients with Crohn’s disease (P=0.008). The most frequent risk factors for cardiovascular disease were a sedentary lifestyle (83.8%), abdominal obesity (52.5%) and excess weight (48.8%). Conclusion The results of the complete anthropometric evaluation draw one’s attention to a nutritional paradox, with high frequencies of both - muscle depletion, as well as excess weight and abdominal obesity. .


Contexto Evidências sugerem um processo de transição nutricional nos pacientes com doenças inflamatórias intestinais. A obesidade, que era um distúrbio incomum nas doenças inflamatórias intestinais, cresce em paralelo com a prevalência da obesidade em toda a população, podendo aumentar o risco de doenças cardiovasculares nesta população. Objetivo Avaliar o estado nutricional e a ocorrência de fatores de risco cardiovascular em pacientes com doenças inflamatórias intestinais. Métodos Métodos – Estudo transversal, série de casos, realizado em pacientes ambulatoriais com doenças inflamatórias intestinais, adultos e de ambos os sexos. Foram obtidas variáveis demográficas, socioeconômicas, clínicas, antropométricas e os fatores de risco cardiovascular: sedentarismo, excesso de peso, obesidade abdominal, medicamentos, comorbidades, alcoolismo e tabagismo. O nível de significância foi de 5,0%. Resultados A amostra foi constituída por 80 pacientes com doenças inflamatórias intestinais, 56 (70,0%) com etocolite ulcerativa inespecífica e 24 (30,0%) com doença de Crohn, sendo a idade média 40,3±11 anos e 66,2% do sexo feminino. Elevadas frequências de excesso de peso (48,8%) e obesidade abdominal (52,5%) foram identificadas, segundo o índice de massa corporal e a circunferência da cintura, respectivamente, em ambos os grupos, e de forma mais importante na retocolite ulcerativa inespecífica. A depleção de massa muscular ocorreu em 52,5% dos pacientes segundo a circunferência muscular do braço, sendo mais depletados aqueles com (P=0,008). Dentre os fatores de risco para doenças cardiovasculares, os mais frequentes foram o sedentarismo (83,8%), o perímetro abdominal elevado (52,5%) e o excesso de peso (48,8%). Conclusão A avaliação antropométrica completa mostrou elevada frequência de déficit proteico somático ao mesmo tempo em que se encontraram elevadas frequências de excesso de peso e obesidade abdominal. .

ABSTRACT

A sarcopenia é uma condição crônica associada ao processo fisiológico de envelhecimento e é definida pela redução da massa, força e função musculares. Na Doença Renal Crônica (DRC), a sarcopenia é prevalente, e associa-se ao aumento da morbimortalidade e à ocorrência de complicações cardiovasculares. Ao analisarmos a sarcopenia em pacientes com insuficiência renal, destacam-se mecanismos complexos que contribuem para a perda de massa muscular, como ativação de mediadores que estimulam o sistema da ubiquitina-proteossoma (SUP) dependente de ATP, inflamação, acidose metabólica, angiotensina II e alguns fatores hormonais. A abordagem terapêutica da sarcopenia na DRC inclui a realização de exercícios, correção da acidose metabólica, reposição hormonal e tratamento da resistência insulínica. Desta forma, é de suma importância o reconhecimento precoce da sarcopenia nesta população, com o intuito de estabelecer intervenções terapêuticas eficazes, evitando-se, assim, toda a gama de complicações associadas à perda de massa muscular na DRC.


Sarcopenia is a chronic condition associated with physiological aging process and is defined by the reduction of the mass, muscle strength and function. In Chronic Kidney Disease (CKD), sarcopenia is prevalent and is associated with increased morbidity and mortality and the occurrence of cardiovascular complications. By analyzing sarcopenia in patients with renal insufficiency, complex mechanisms that contribute to loss of muscle mass are highlighted, such as activation of mediators that stimulate the ubiquitin-proteasome system (SUP) ATP-dependent, inflammation, metabolic acidosis, angiotensin II and some hormonal factors. The therapeutic approach to sarcopenia in CKD includes exercises, correction of metabolic acidosis, hormone replacement therapy and insulin resistance. Thus, it is of paramount importance early recognition of sarcopenia in this population, in order to establish effective therapeutic interventions, thus avoiding the full range of complications associated with muscle wasting in CKD.
